All Girls Ball Skills Camp

ASSUMPTION OF RISK I, the undersigned parent or legal guardian, confirm that I am the legal guardian of the minor named in this registration form. I hereby grant permission for my child to participate in the All Girls Ball Skills Camp with Coach D held on July 11, 2026 at The Zone, 10371 Stella Link Rd, 2nd Floor, Houston, TX 77025. I fully understand and acknowledge that participation in basketball and related athletic activities involves inherent risks of injury, including but not limited to: falls and contact with other participants, hard surfaces, and equipment; sprains, fractures, concussions, and other physical injuries; heat-related illness or exhaustion; and unforeseen accidents or emergencies. I voluntarily assume all such risks on behalf of my child. RELEASE AND WAIVER OF LIABILITY I hereby release, waive, discharge, and covenant not to sue All Girls Ball, its officers, directors, employees, coaches, volunteers, agents, and representatives (collectively "All Girls Ball") from any and all liability, claims, demands, causes of action, or damages of any kind arising out of or in any way connected with my child's participation in this event, whether caused by the negligence of All Girls Ball or otherwise. INDEMNIFICATION I agree to indemnify, defend, and hold harmless All Girls Ball from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or related to my child's participation in this event, including any claims brought by third parties. MEDICAL AUTHORIZATION I confirm that my child is in good physical health and is medically able to participate in athletic activities. I authorize All Girls Ball and its representatives to consent to and obtain emergency medical treatment for my child if I cannot be reached. I accept full financial responsibility for any medical expenses incurred. I acknowledge that All Girls Ball does not provide accident or health insurance for participants. I confirm that my child is covered by my own health insurance or that I accept full financial responsibility for any medical costs. PICK-UP POLICY I understand that my child must be picked up between 12:30 PM and 1:00 PM on July 11, 2026. A valid photo ID is required at pick-up. If someone other than the parent/guardian listed on this form will be picking up my child, I have provided their information in the authorized pickup section below. All Girls Ball reserves the right to refuse release of a child to any unauthorized individual. PHOTO & VIDEO RELEASE I grant All Girls Ball permission to photograph and/or video record my child during this event and to use such images for promotional, educational, and social media purposes without compensation. CODE OF CONDUCT I agree that my child and I will conduct ourselves in a respectful and sportsmanlike manner. I understand that All Girls Ball reserves the right to remove any participant or guardian who engages in inappropriate, unsafe, or disruptive behavior, without refund. REFUND & CANCELLATION POLICY Registration fees are non-refundable. Refunds will only be issued in the event that All Girls Ball cancels the event, in which case a full refund will be provided. GOVERNING LAW & SEVERABILITY This agreement shall be governed by the laws of the State of Texas. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect. This agreement is binding on me and my heirs, assigns, and legal representatives. By checking the box below, I confirm that I am the legal parent or guardian of the registered athlete, that I am at least 18 years of age, and that I have read, fully understood, and voluntarily agree to all terms of this waiver and release of liability.
